  1. Individual applicants: licensing examinations: Contact Prometric for examinations. Arizona law does not require prelicense education. Some individuals are required to pass a prelicense examination as one of the qualifications for becoming licensed. Individuals applying for all major lines of authority must successfully pass the Arizona Producer licensing exam(s). Arizona does not allow any professional insurance designations as exemptions to licensing examinations.
  2. Any and all applicants that are pended for state review will receive in addition to their specific reasoning for being pended this message. "Based on an automated review of the application, the State of Arizona has determined this application to be administratively deficient. Arizona law requires license applicants to submit proof of lawful presence in the United States . In order to process your application Form L-152 must be submitted.   3. Individual applicants are required to send the following attachments to the Arizona Department of Insurance, Producer Licensing section, ATTN: electronic resident licensing:
    a. All individual resident applicants and all adjuster applicants: submit fingerprint card
    b. For variable Life/Annuity agents, submit copy of CRD report showing individual is registered for securities with the FINRA (and must hold an active Arizona Life Insurance License) Note: This is applicable for both licenses
  4. Effective January 1, 2005, a license issued to an individual will expire on the last day of the individual's month of birth. License fees shall not be prorated. Surplus line Fees are the only prorated fees.
  5. An individual who relocates to Arizona from another state is required to pass a prelicense examination if residency exceeds the 90 days of cancellation of the applicant's previous license, A.R.S §20-288(A)(2).B246
  6. The following must be supplied, if an individual answered yes to one or more questions:
    a. A signed statement describing (in detail) all incidents including
    1. Names of all involved.
    2. Dates and locations.
    3. The names and localities of any courts and/or administrative agencies involved.
    4. The disposition of each matter.
    5. Whether the conviction, plea or finding was for a felony, misdemeanor or open-ended charge.
    b. Attach copies of any of the following:
    1. All indictments,
    2. Complaints,
    3. Plea agreements,
    4. Orders of conviction,
    5. Notices of hearing or trial,
    6. Sentencing orders,
    7. Suspension/revocation orders and other information, which relates to each matter.
  7. EFT Customization – Report must distinguish funds for Surplus Lines Broker and Surplus Lines, fingerprints, Mexican Ins. Deposit can be made to one account.
  8. Arizona law requires a licensee to report the following within 30 days
    a. A change to a licensee's residence or business address [A.R.S. § 20-286(C)] -- please complete and send the Licensee Information Change Notification (Form L-CHG);
    b. A change to an individual's or business entity's name (A.R.S. § 20-286(C) as amended by Laws 2004, Chapter 162) -- please send appropriate evidence that the name has legally been changed. A business entity must provide a copy of relevant Arizona Corporation Commission or Arizona Secretary of State approval documents.
    c. After the final disposition of the matter, an administrative action taken against the producer in another jurisdiction or by another governmental agency [A.R.S. § 20-301(A)] -- you are required to provide a written statement accompanied by a copy of the order, consent to order or other relevant dispositive document;
    d. After the initial pretrial hearing date, a criminal prosecution of the producer in any jurisdiction [A.R.S. § 20-301(B)] -- you are required to provide a written statement accompanied by a copy of the initial complaint filed, the order resulting from the hearing and all other relevant legal documents.
