Original Resident Application:

  1. Individual applicants: licensing examinations: Contact Promissor except for Title examination, which is administered by licensing staff. Individuals applying for all major lines of authority must successfully pass the Hawaii Producer licensing exam(s). Hawaii does not allow any professional insurance designations as exemptions to licensing examinations.
  2. An appointment form need not be submitted with the application. However, refer to 431:9A-114 Appointments: (a) an insurance producer shall not act as an agent of an insurer unless the insurance producer becomes an appointed agent of that insurer or is contracted with and appointed by an insurance producer so appointed.
  3. Licensees are reminded to secure a General Excise Tax License, as required under the tax laws for the State of Hawaii. Please contact the Hawaii Department of Taxation at (808) 587-4242 or call (800) 222-3229 for more information.
  4. All applicants: must supply a Hawaii resident or business address.
  5. Individual applicants are required to send the following attachments to the Hawaii Department of Commerce and Consumer Affairs, Insurance Division, Producer Licensing: All individual applicants: submit Criminal History Background Check For variable products applicants, submit copy of CRD report showing individual is registered for securities in Hawaii with the NASD (and must hold an active Hawaii Life Insurance License)
  6. Business entity/agency applicants are required to send the following attachments to the Hawaii Department of Commerce and Consumer Affairs, Insurance Division, LICENSING BRANCH. Has designated a licensed producer who is a natural personal responsible for the business entity's compliance with the insurance laws and rules of this State. Any licensed producer so designated or empowered by a corporation or partnership may not be so designated or empowered by more than one corporation or partnership, except when the corporation or partnerships are affiliates of each other. As used herein, a corporation or partnership is an affiliate of another corporation or partnership, if the same person directly or indirectly through one or more intermediaries, controls both corporations or partnership. All business entity applicants: submit evidence of approved registration with state Department of Commerce & Consumer Affairs Business Registration Division. All business entity applicants: submit a copy of filed Articles of Incorporation. As filed with the Business Registration Division. All business entity applicants for Variable Products license: must already have an active HI "Life" LOA license on the PDB or be applying for a "Life" license at the same time
