STATE SPECIFIC REQUIREMENTS

• Applicants who provide an affirmative answer to any of the background questions should fax a written, signed explanation, along with court documents, for each incident in connection with that answer. Failure to supply the necessary documents within 90 days of application will result in denial of the application.
• Individual applicants – Applicant must be 18 years old or older as determined from the applicant’s Date of Birth.
• All applicants cannot apply earlier than 45 days prior to their license expiration date.
• Licenses are good for two years; Individuals will renew at the end of their birth month and business entities will renew on the 1st of the month following the month of issue.
• Individual applicants may not reinstate an inactive non-resident license.
• Business Entity applicants – Must be a Legal Business Type.

  • 4 – Corporation
