STATE SPECIFIC REQUIREMENTS

• Business Entities: Please confirm through PDB that each Designated/Responsible Licensed Producer does hold an active license with the required authorities and at least one active appointment. Kentucky business entity applications must be submitted separate from other state application requests.
• When completing a business application, you may be required to complete the following:
• Organizational Documents: Partnership Agreements and Certificate of Registration from KY Secretary of State (if applicable)
• Certificate of Assumed Name from KY Secretary of State and each KY County Clerk where Business Entity will do business, if using a "Doing Business As"(Dba Name) (if applicable)
• Certificate of Authorization from KY Secretary of State's Office, authorizing Business Entity to operate in the State of KY (if applicable)
  Supporting documentation for the above can be located at www.sos.state.ky.us.
• To avoid penalties and/or fines, immediately change address by utilizing Kentucky
  e-Services at: https://doi.ppr.ky.gov/kentucky/secured/Eservices/default.aspx

State fees will be collected by NIPR (on behalf of KY) when a transaction is declined according to KY business rules. Transaction fees and processing fees will also apply. Violation of the following rules will result in a decline response.

• Individual applicants - Applicant must be 18 years old or older as determined from the applicant’s Date of Birth.
• Non-Citizen of U.S. Must hold U.S. resident license.
• Individual applicants - Applicant must enter NASD CRD number on application for Variable Products.
• Applicants applying for Surplus Line Broker Licenses must actively hold Property LOA in NR state.
• Individual applicants - Applicant applying for Variable Products must also apply for or have Life LOA in KY.
• Business Entity applicants - Must be a Legal Business Type. 
• 1 - Partnership
• 2 - Limited Liability Company
• 3 - Limited Liability Partnership
• 4 - Corporation

• All ADJUSTER licenses will require either the minimum $1,000 adjuster surety bond (Form 99-3) or ADJUSTER Insurer Assumption (Form 99-6) for both residents and non-residents. The documents must be issued by a Kentucky admitted insurer and must contain original signatures. Copies of the forms are not accepted.  All ADJUSTERS are required to have an active surety bond on file as long as the license is active.
• All KY ADJUSTER applicants must be at least 21 years of age.
