STATE SPECIFIC REQUIREMENTS
- Applicants who are currently employed by, own stock in, are an officer or director of, or are in any other manner connected with a funeral establishment, mortuary, or cemetery cannot hold an insurance license in Michigan except the limited life license.
- Applicants may add an additional LOA to an existing active non-resident license.
- Applicant must be 18 years old or older as determined form the applicant’s Date of Birth.
- Applicants are allowed to use P.O. Boxes for mailing addresses. A physical address is required in the residence/home address only.
- Upon answering any of the background questions with a “Yes” response, please send
- A written statement explaining the circumstances
- A certified copy of the official document which demonstrates the resolution of the charges or any final judgment.
- Individual applicants license numbers are their SSN.
