On Monday, May 12, 2008, the Ohio Department of Insurance will begin operations at its new office in the renovated Lazarus building, located in Downtown Columbus.

The Department will have limited services and availability on Friday, May 9, 2008, as we transfer equipment and personnel to the new facility. The consumer, fraud and OSHIIP hotlines will be operating that day. On May 9, 2008, you will still be able to reach the Department through our main switchboard phone number, 614-644-2658. More information about our move can be found on our web site, www.ohioinsurance.gov.

STATE SPECIFIC REQUIREMENTS

Business Entities: All business entities, except partnerships, may need to register with the OH Secretary of State prior to operating in the state of OH
Designated Producer(s) are required to hold an OH license (resident or NRL).
Business Entities: Non-resident business entities are not required to register with the Ohio Secretary of State prior the Department issuing an insurance license. However, the Ohio Secretary of State may still require registration of the non-resident business entity prior to the entity operating in the state of OH.

State fees will be collected by NIPR (on behalf of OH) when a transaction is declined according to OH business rules. Transaction fees and processing fees will also apply. Violation of the following rules will result in a decline response.

Applicant must not have an inactive Non Resident license in OH.
Individual applicants - Must supply NASD CRD # on application when applying for Variable.
