  • Fees shown are state fees only. These fees do not include NIPR transaction fees.
  • Fees are per application and are deemed earned when received and are not refundable, per KRS304.9-200(4).
  • An individual submitting insurance application or renewal fees who is the spouse of a current member of the Armed Forces of the United States who is assigned a duty station in the Commonwealth of Kentucky shall be eligible for refund of fees, pursuant to KRS 12:357.  Upon notification to the Kentucky Department of Insurance, the fees will be refunded to the individual within thirty (30) days of their submission if the applicant demonstrates compliance with the requirements of KRS 12:357.
    • To demonstrate compliance with KRS 12.357, an applicant shall include the following with their application:

1.    Proof that the applicant is married to a current member of the Armed Forces of the United States;

2.    Proof that the applicant's spouse is assigned to a duty station in the Commonwealth of Kentucky; and

3.    Proof that the applicant has satisfied all statutory and regulatory pre-licensing course of study requirements, and all statutory and regulatory examination requirements, or proof that the applicant holds a valid individual insurance license issued by another state, the District of Columbia, or any possession or territory of the United States.

  • Reinstatements:
    • If applicant is applying within twelve (12) months of license termination, the exam fee is waived for inactive lines of authority (LOAS) held. Regular license fees apply and there will be no additional reinstatement fee.
    • If all lines of authority are inactive the class fee will be assessed. If one (1) or more LOAS are active, then the class fee will be waived.
      • Example: Individual producer reinstating life and variable license would have a reinstatement fee of $40.00 (class fee) + $40.00 (life LOA) + $40.00 (variable LOA) = $120.00.
    • If all LOAS are inactive the class fee will be assessed. If one (1) or more LOAS are active, then the class fee will be waived.
      • Example: Individual producer reinstating life and variable license would have a reinstatement fee of $40.00 (class fee) + $40.00 (life LOA) + $40.00 (variable LOA) = $120.00.
  • Amendment: 
    • Regular license fees apply.
      • Example: Individual producer adding life license would have an amendment fee of 
        • $40.00(life LOA) + $50.00 (life exam) = $90.00
  • Reinstatement and Amendment:
    • If all lines of authority (LOAS) are inactive, class fee will be assessed. If one (1) or more LOAS are active, then class fee will be waived.
      • Example: Individual producer reinstating life and variable license plus adding new line of property would have a reinstatement fee of $40.00 (class fee) + $40.00 (life LOA) + $40.00 (Variable LOA) = $120.00. Amendment fee would be $40.00 (property LOA) + $50.00 (exam fee) = $90.00. Total reinstatement and amendment fee would be $210.00.
    • If all LOAs are inactive, class fee will be assessed. If one (1) or more LOAs are active, then class fee will be waived.

Reinstatement and Amendment Example:

  • Individual producer reinstating life and variable license plus adding new line of property would have a reinstatement fee of $40.00 (class fee) + $40.00 (life LOA) + $40.00 (Variable LOA) = $120.00.
  • Amendment fee would be $40.00 (property LOA) + $50.00 (exam fee) = $90.00. Total reinstatement and amendment fee would be $210.00.

Business Rules

  • Applicant must be eighteen (18) years old or older as determined from the applicant's date of birth, with an exception for Administrator applicants, who must be at least twenty-one (21) years old.

  • Applicants who are non-citizens must provide a copy of work authorization or green card.

  • Applicant must hold or apply for life to hold a variable line of authority (LOA).

  • Applicant must actively hold or apply for property and casualty LOA to hold a surplus lines broker license or a managing general agent license.

  • Applicant cannot actively hold independent adjuster license in conjunction with public adjuster, or staff adjuster license.

  • Applicant cannot hold public adjuster license in conjunction with staff or independent adjuster license.

  • Applicant cannot hold staff adjuster license in conjunction with public or independent adjuster license.

  • Applicants applying for the variable LOA must supply FINRA CRD number on application.

  • To apply for life agent registered as life settlement broker license, applicant must have held an active agent life license for at least one (1) year.

  • All reinsurance intermediary broker and managers must provide list of insurers to be represented. The state will verify agent or surplus lines broker license needed to hold this license.

  • Applicant with CE affidavit on File may not amend.

  • If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, email, or fax.
