Effective January 1, 2008, Missouri-resident producers licensed for any major line of authority will be required to complete sixteen (16) hours of continuing education during any two-year licensure period. Please also be aware that the Department has expanded the options by which to comply with this law by including classes completed and passed as part of a Department-approved professional financial designation study program*** |

| Organizational Documents |
|
|
Domestic Corporations, Limited Liability Companies, or Limited Liability Partnerships: Attach a Certificate of Good Standing, Certificate of Incorporation, or a Certificate of Organization. The Certificate must be dated within the past year and issued by the Missouri Secretary of State. |
|
Foreign Corporations, Limited Liability Companies or Limited Liability Partnerships: Attach a copy of the Certificate of Good Standing, Certificate of Incorporation or Certificate of Organization. The Certificate must be dated within the past year and be issued by the state granting the authority to conduct business as a corporation or limited liability company or by the Missouri Secretary of State. If the business entity has an office in the state of Missouri, the certificate must be issued by the Missouri Secretary of State. In lieu of a Secretary of State document, we will accept a letter of certification dated within the past 6 months from your home state insurance department. |

Partnerships: Attach a copy of the Registration of Fictitious Name issued by the Missouri Secretary of State. |
|
Sole Proprietorship: A Registration of Fictitious Name is not required. |

List all Missouri licensed Insurance Producers conducting business on behalf of the Business Entity Producer. Include the producer's legal name and License or Social Security Number. The Business Entity Producer must have at least one Missouri licensed producer at each place of business and the application will be returned if this listing is not attached. |

If paper application is submitted, you
must submit the completed Uniform Application for Business
Entities along with all of the requirements listed above and
attach a company or agency check, cashiers check or money
order for $100.00. Checks may be made payable to the
Missouri Department of Insurance, Financial Institutions and
Professional Registration. Personal checks are not

If application was submitted electronically,
do not submit an additional fee to the Missouri Department

| Producer Licensing Requirements |

Residents applying for a Producer license may contact Pearson VUE at 866-274-4740 or online at http://pearsonvue.com/ to register for the Missouri examination. All Missouri exam scores are valid for 1 year. |

Non-Residents must attach an original Letter of Certification (no older than 6 months). A Letter of Certification is not required if the applicant’s resident state participates in the PDB (PDB Participating State Report can be found at: http://www.nipr.com/pdb.htm). |
|
Complete and submit the Uniform Application for Individual Non-Resident License. |
