Re-instatement applications on the NAIC Uniform application plus re-instatement fee of $50.00. (Paper applications are only accepted for those applicant's previously licensed in SD as the systems are blocking re-instatement applications. This is caused by the different fee needed. As soon as this blockage is cleared up all applications will need to be submitted electronically.) |
