Appointment Requirements
 
 
Appointments are to be done electronically by the Insurer. Please contact your in-house licensing administrator for further instructions.
Other Requirements
 
Attach a check or money order for appropriate amount. The new agency fee is $87.00. Checks payable to Utah Insurance Department
 
A foreign entity must register with the Department of Commerce prior to applying to the Insurance Department
 
Include Letter of Certification (within 90 days of issuance)
 
Current PDB Screen print outs are acceptable in place of Letter of Certification
 
In reference to question 23 on the business application, Utah does not require that the agent designated to act on behalf of the business entity be an officer or partner in the business entity.  The designee need only be licensed as a licensed producer in the state.

Utah's preferred method of application for Non-Residents is electronic. With the exception of Limited Licenses, Adjusters, TPA's and Consultants, all licenses can now be applied for on-line and paid for with a major credit card. Failure to utilize this method will result in a $25 paper processing fee and a longer processing time for the applications.

Include Letter of Certification (within 90 days of issuance)

NASD Exam/Status requirements:  Applicant must include a recent CRD (30 days old or less) which shows that the applicant is registered to sell Securities in Utah.  The CRD can be obtained from the broker/dealer.
