If your resident license information  is not available on the NAIC Producer DataBase (PDB), include Letter of Certification* (within 90 days of issuance).
 
Provide proof of passing series 6, 6.3 or 7, for variable annuity requests

MGA's: E&O in the amount of $250,000
Appointment Requirements
 
Sponsoring Insurance Company must submit Uniform Appointment/Term Form within 15 days from the date the agency contract is executed or the first insurance application is submitted, whichever is sooner.  Billings for appointments are sent monthly to sponsoring companies.
