Submit Articles of Incorporation or Articles of Organization
 
Firms and Trade names must be filed with Business License Center, Trade Names Division (360-664-1400)
Appointment Requirements
Appointment is required at time of application with any insurer to be represented unless the appointment(s) and fee are going to be submitted electronically through NIPR within thirty (30) days of the acceptance the first insurance application executed by the agent with the insurer.

Affiliation required for all individuals who represent the business entity in its transactions in Washington

 
Supply INS-18, Appointment/Affiliation Notice ( www.insurance.wa.gov/)
 

Appointment fee is $20 each, payable to the Office of Insurance Commissioner (OIC).

 

 Affiliation fee is $20 each, payable to the Office of Insurance Commissioner (OIC)

Financial Responsibility: Broker license only

 

$20,000 Broker Bond - Form INS-17( www.insurance.wa.gov/) (covers brokerage and one affiliate - $5000 increase for each additional affiliate)

Other Requirements

 

Filing Fee $5, payable to the Office of the Insurance Commissioner

 

License Fee $50 for agents or $100 for brokers, payable to the Office of the Insurance Commissioner (all fees payable to the Office of the Insurance Commissioner may be combined into one check)

 

Include a Letter of Certification within 180 days of issuance and not to expire within 30 days. If resident state does not license business entities, must submit a Certificate of Good Standing from resident Secretary of State (not older than 180 days)

 

In reference to question 23 on the business application, Washington does not require that the designated/responsible licensed producer be an officer, director or partner of the business entity.

Submit 1 completed fingerprint card, Form #FD258 with a $49.25 check payable to WA State Patrol, ID Section (no personal checks accepted)
