Applicant Can

Business Rules

  • Applicants must apply for the License Classes and Lines of Authorities they qualify for. If you apply for a License Class or for Lines of Authority that you do not qualify, the Department is unable to refund your fees. ARS 20-167.

  • If applicant has an Arizona resident license that is active or in the late renewal period (up to one year past license expiration date), applicant will be rejected from applying for a Non-Resident License.

  • Applicant's birth date must match current PDB record for resident state (if PDB birth date is blank this is accepted).

  • Applicant's name must match current PDB record for resident state (if applicable)

  • Applicant must be 18 years old or older as determined from the applicant's Date of Birth.

  • Variable Life and Variable Annuity applicants - If you intend to sell both fixed and variable life/annuity products you must also hold a Life line of authority (LOA) in addition to Variable in Arizona.

  • P.O. Box or PMB (Private Mailbox) cannot be used for either residence or business address.

  • All instances of phone number must be a valid 10 digit phone number.

  • Life Settlement Broker applicants are required to hold the Life line of authority (LOA).

  • Applicants applying for Navigator or Certified Application Counselor must provide training certification from United States Department of Health and Human Services. to Insurance Licensing Section 100 North 15th Avenue, Suite 102 Phoenix, AZ 85007-2624 or email to licensing@azinsurance.gov. Please include your NIPR Transaction Number.

  • Surplus Lines Broker Applicants - If you intend to sell non-surplus lines property and casualty products you must also hold the Property and/or Casualty lines of authority.

  • Applicants applying for Navigator must have at least one entry on the Affiliations section of the application that is actively licensed as a Navigator in Arizona.

  • Applicants with a status reason of surrendered cannot reapply for one year from date of surrender. After one year applicant must reapply as new.

  • Applicant's Non-Resident (active or inactive) AZ license must not have the following status reasons:
    • Suspended
    • Revoked
    • Deceased
    • Refuse to Renew
    • Military
  • If applicant responds "yes" to one of the background questions please submit supporting documents electronically via NIPR's Attachments Warehouse. The documents may also be submitted to the state via mail, or email.

  • If you intend to do business under a name other than your legal name (called an "assumed name" or "DBA name"), and did not already disclose this name on the DBA section of the application, complete and submit Form L-193 (Certificate of Assumed Business Name).

  • Protect your assumed name by registering it as a trade name with the Arizona Secretary of State. If you do not register a trade name, another person may register the name, which will force you to stop using the name

  • Rights and Responsibilities:
    • Licensing time frames. Once the Arizona Department of Insurance receives the application, it has up to one hundred and twenty (120) days to determine whether it contains all required information and is therefore "administratively complete," and up to an additional sixty (60) days to review the substance of the application. These timeframes are the outer limits by which the Department must grant or deny a license application. The Department processes most applications within thirty (30) days.
    • Once your application is submitted to the Department, you have 60 days to cure any deficiencies on the application or the application will be administratively withdrawn and the fees forfeited.
